I haven't even seen the form so can't answer this unfortunately! Is
there a question about being able to cook meals (you could, if it's
true for you, say that you don't have the motivation to cook and the
depression means that the last thing you want to do is eat) or go food
shopping (again, if it's true for you, you could say you don't have the
motivation to go shopping) or you could say (again, if it's true for
you) that you don't leave the house unless you really have to - things
like that?

Just write the word "cunt" in answer to each question, they will soon
realise you are depressed. Only joking, it makes no difference how you fill
in the form, well maybe it makes a little bit of difference. Make your
answers as confusing and off the wall as possible, Atos will declare you fit
for work regardless and you will have to appeal. Then the form can be used
as evidence at tribunal to show you are totally insane. ;)
